It is likely that they will cap the amount that is paid out for a specific treatment per year. In addition, the insurance company will also cap the maximum that will be paid out per year for any treatment. You will have to pay for the treatment up front. Following this, you can claim the money back by completing a form that is countersigned by your surgery or clinic.
