Thank you for your post. I know of several really good Implantologists who can provide you with a bone graft, soft tissue graft and an implant.
Here are details of two good implantologists with a lot of experience:

My insurance people said they cover "unerupted teeth".
If that means they cover the extraction, or the treatment..I am not sure. I had a surgeon in sydney, who advised I have it removed, and then an implant placed. Do you not think any aspects (even claiming back what was spent on extraction) could be claim-able?
With some aspects of the treatment it might be possible to claim back from the insurance company, such as unerupted tooth extraction, temporary denture,...
However its best to check this with your insurance company as the policies vary from company to company and some insurance companies insist on extractions by specialist maxillofacial surgeons and so on.

When you say a specialist, do you mean they choose? Or do you mean a malfixation surgeon in general? Because my surgeon was? Do you not think if my tooth was unerupted, and ankloysed and the surgeon and orthodontist suggested an implant it would be covered?
Also the health cover is a premium health insurance if that makes a difference?
Sounds like your extraction should be covered as long as your insurance policy covers oral surgery and most of them do.
When it comes to oral surgery, there are recognised specialities such as Maxillofacial surgery and oral surgery and with such complex extractions, some insurance companies insist that the extraction is carried out by these specialists.
If you contact your chosen insurance company, they will be able to tell you the exact terms of your policy with regards to such things.

If I want to get a referal, can I do this through my dentist?
Can a block bone graft, if the bone is taken from my jaw/chin, be done under local?
Any other good surgeons?
Your dentist can usually do the referal to a specialist for you.
Block bone grafts from the jaw and the chin are usually carried out under local anaesthetic but can also be done under sedation if you wish.
I don't know of any other really good surgeons other than the two mentioned. Those two surgeons that I mentioned, I have actually seen them work and both are very good indeed.

I checked with my insurance, and unfortunatly since I had the work done overseas, nothing is clameable. V. annoying!
How much would you estimate a block bone graft and implant (does that include the crown?) and possibly a gum graft in the area (I have alot of what looks like white scar tissue in the area from numerous ops) to be @ The Pearl Cosmetic?
And would you advise the payment plan over a few years?
Oh & Also - do costs on the site cover local anesthetic used during operations?
